Privacy Request

1. Types of requests you can make

Depending on your state of residence, you may have the right to:

• Access — receive a copy of the personal information we hold about you.
• Delete — ask us to delete personal information we hold about you, subject to legal exceptions.
• Correct — ask us to correct inaccurate personal information.
• Opt out — opt out of the sale or sharing of personal information, or of certain targeted advertising.
• Limit use of sensitive information — limit the use and disclosure of sensitive personal information to what is necessary to provide the services you requested.
• Appeal — appeal our decision if we deny a request in whole or in part.

2. How to submit a request

We do not provide an online form for privacy requests. To submit a request, email [email protected] with the subject line “Privacy Request”. Include:

• Your full name
• The phone number and email address you used when you submitted a request on this website (if any)
• Your state of residence
• The type of request (access, delete, correct, opt out, limit sensitive use, or appeal)
• A brief description of what you are asking us to do

We will respond within 45 days. If we need more time, we will let you know within that period and may extend the response time by an additional 45 days where permitted by law.

3. Verification

Before fulfilling a request, we must verify that you are the person whose information is involved or an authorized agent. We may ask for additional information to confirm your identity. We will only use that information for verification.

4. Authorized agents

You may designate an authorized agent to submit a request on your behalf. The agent must provide proof of authorization (such as a signed permission letter or power of attorney) and we may still require you to verify your identity directly.

5. Appeals

If we deny your request in whole or in part, you may appeal. Send an email to [email protected] with the subject line “Privacy Request Appeal” and include the original request details and the reason for your appeal. We will respond to the appeal within 45 days.
